To obtain a disciplinary history letter, send a written request (by mail, email or fax), signed by the requesting attorney, to:

Your request should include your full name, lawyer registration number, the purpose for which the letter is needed, and the address to which it should be sent.
Please note that /s/ signatures will not be accepted. Verified electronic signatures are accepted. There is no fee associated with this request.
