​Instructions for Filing a Complaint 

There are two versions of the English ​complaint form available on this web site. You may
print and complete the complaint form and mail or fax to our Office, or you may file a complaint online (see below).

PLEASE DO NOT SUBMIT ORIGINAL DOCUMENTS.  The OLPR has moved away from paper files to electronic files.  Paper documentation will be scanned by the OLPR and then securely shredded.  If you must submit original documents, please note that fact with your submission and request the original document be returned.   ​

If you are submitting documents with your Online Complaint, you can email those documents to OLPRComplaintDocs@courts.state.mn.us
